Waiver Policy

Parents and students are asked to agree to the following upon registration for classes:

I acknowledge that participation in the activities for which I or my child is registered includes physical activity and involves possible risks of injury with or without the negligence of others. Students and their parent/legal guardian assume such risks as a condition to participation in classes and activities administered by Twin Cities School of Dance. I authorize Twin Cities School of Dance to obtain medical treatment for the student when appropriate should injury occur. I waive all claims, release, discharge and agree to indemnify and hold harmless Twin Cities School of Dance, as well as its officers, employees and agents from any and all liability for personal injuries and damage to property arising out of the activity for which the student is registered.

I acknowledge and accept that the student’s participation in activities includes the necessity for the instructor to adjust and correct the student’s body form.  I am aware that the instructor may, at times, touch the student to correct body position, and I agree such action is necessary and appropriate.  I agree to release and hold harmless Twin Cities School of Dance, as well  as its officers, employees and agents from any liability as a result of any such action.  I also agree to indemnify Twin Cities School of Dance from any litigation expenses, attorney fees and damages as result of any such claim.

I acknowledge and accept that there are no refunds of any fees, for any reason, unless initiated by Twin Cities School of Dance. I also understand checks returned from financial institutions will incur a $30.00 fee and a $15.00 late fee charge will be incurred when payments are not received by the 7th day of the month in which they are due.

I agree that payments are due on time according to the payment schedule set forth on the TCSD website, based on the payment option I selected (annual, semester, or installments). Should payment be late more than 30 days, I understand that my student will not be allowed to participate in class until payments are made current.*

I acknowledge and agree that I and the student will abide by Twin Cities School of Dance Discipline Policy and general studio rules.

I understand performances and classes may be photographed and/or videotaped.  I grant Twin Cities School of Dance exclusive right to utilize any and all photographs and/or video footage for advertising, promotional or any other purpose.

I, on my and the student’s behalf, acknowledge I have read and understand this document and its contents.

*Should you experience a financial hardship, please speak with Vice President Jeannie Phillips to determine payment options.
